The opportunity:
To ensure a person centred and high-quality service to meet the participants needs and scheme
objectives. In the role you will be able to handle sensitive conversations and documents in supporting the Aligned
staff to carry out the duties of being a support worker. You will understand how disability impacts on daily living
and identify how the individual would benefit from support strategies.
It’s an exciting time at Aligned community Care as we are currently seeking a Service case officer. At Aligned no
two days are the same; as a Service case officer you will:
• Monitor a caseload of NDIS participants who suffer with complex mental health and other disabilities
• Work with management to ensure appropriate strategies are provided according to participant’s needs.
• Use a person-centred approach to assist in identifying and reviewing the needs of participants
• Create content when needed as well as facilitate staff training to enhance support workers awareness of
each participant.
• Work collaboratively with health professionals and other key stakeholders to coordinate the most
effective options to meet the care and support needs of the participant.
• Apply critical reasoning and thinking within a decision-making framework with the ability to make
recommendations for reasonable and necessary strategies from care and support plans
To be considered for the role of Service case officer with Aligned Community Care the ideal candidate will meet
the following key Competency
▪ The Candidate will have the ability to read and summarize clinical reports produced by doctors,
psychologist, and other related parties.
▪ The ability to develop Client Care Plan’s from the summarization of clinical reports
▪ Ability to identify data collection needs and develop appropriate questioner to satisfy the data collection
needs
▪ Excellent Interpersonal skills with the ability to communicate clear and effectively with Aligned staff and
stakeholders
▪ The ability to create content for training material as well as train and support staff
▪ Proficiency in Microsoft Office software
▪ Is a highly motivated self-starter, with proven ability to develop creative solutions
▪ Able to multi-task and remain calm in demanding and unpredictable situations
▪ Display a high level of initiative, effort, and commitment towards completing tasks efficiently
▪ Possess excellent time management skills and the ability to work with minimum supervision
